I tightened the head bolts
 
Exhaust rocker now has a gap of .25...............it got better, at torq spec

The middle screw is the adjuster.  The nut just LOCKS IT IN PLACE.
 
Intake should be closer to .006", exhaust about .008"
 
Crack the nut loose, adjust the center part to spec, lock down the nut (good and snug).

There is no middle screw. Just one nut, for each rocker

Ok, part #192 is the adjuster and locking screw.

As you stated, you have to have that set screw to lock down the adjuster (6 sided nut)...

Yes, those are the actual adjusters. 
 
The little, internal screws are the set screws that lock the unit from moving once adjusted.
